I played this game alot on my Voodoo1 but the 3dfxupgrade did not work on my VoodooČ, even with the v2-auto.inf autoexec.bat variables for compatibility. I searched the internet and found a VoodooČ patch here:

I think you need to install the regular 3dfxupgrade first. This has some more files (i think additional textures for smoke). The carmav2.zip has only two exe's. See readme for details of wich version of carmageddon needs which exe.

I use the final 3dfx patch which you can get here-

This has a number of Voodoo2 specific fixes and a very pretty auto-installer.

The game (and especially the 3dfx version) really needs to be run in DOS to work properly.  It works well here although there are some bugs (minor) relating to joystick configuration.

There is no need to apply the Voodo1 Patch first. The file you linked to is comfortable and works, i checked this with a new install.
