Substantially, you need a compatible mobo, based upon KT333 chipset,
with 5 pci slot (+1 AGP of course) instead of 6 pci slotI have an MSI KT3 ULtra Deluxe, but it doesn't match your goal.
The gigabyte posted by G1nX seems to be what are you looking for, but I don't know if is fully compatible with the v56k.
The right one for you seems to be the Soyo KT333 Dragon Ultra; only 5 PCI slot + 1 AGP slot and tested with Voodoo5 6000
On ebay I am pretty sure you will find something of these
