Hello.
When you remove the Voodoo Graphics Card, the failure is the same?

Hmm...
It's not that easy to do some exact dignostics from distance...
The first I would do for troubleshooting:
Trying an other PCI slot.
Perhaps try to
carefully
bend the card in action, look for some changings in picture. To test it for an intermittend contact.
Then trying an other Voodoo Card.
However, all this ist only guessing - only attempts.
