The one I have is dark blue

It says " Quick Install " on top

and on the bottom it says:

The Voodoo Driver Installation CD

Refer to the Quick Install Guide for your adaptor installation instructions.
This CD contains the following: Windows 95/98 driver, DirectX 7.0 DirectX
Media, E-color 3Deep and True Internet Color.
