I would recommend trying to install the 3dfx Reference Driver for Voodoo4/5. Those (usually) even run on a Voodoo3, or a Velocity, in case I'm in error.
Once 3dfx Tools are installed, you will get precise info what Card Type, Memory, and Speed you are running there.
Theoretically, you could also download only the separate 3dfx Tools Installer from the Tools Section of the Archive. Those installed, it might already give the needed Info Page in the Advanced Display Properties Tab
