definitely indicates a 3dfx Part (BGA).

Of course, I'm just as surprised to see something like this turn up, but all the numbers

add together.
Looking at the very late Production Date and the highest PCB Part No. of the 210-xxx series I've ever seen, it seems quite possible.
Looked at the link a few days ago, 6 VSA-101 chips available. Might be possible to make our own design
