hmm abit on the late side year 1998 week 27 Smiley seen later ones though Wink

The PCB layout looks like from a Quantum3D Obsidian SB50-4220, you can see the mount spots for the connectors of the Amethyst add-on card

skywell also sold 8MB (here a 4MB ) with own layout

Skywell had multiple versions of their Voodoo Graphics card:
