I could not run MK4 in glide mode just D3d. (I looks ok anyway) I read somewhere that requires a fix but I could not find any patch everywhere.

Could be wrong but I remember you could switch to Glide mode by pressing F5, F6 switch to D3D again.

1) t. I already tried F5 and F6 says no 3dfx is present or something similar.  Just detect D3d or software.
I use my voodoo 3 with other software and they detect glide ok.

2) I do not know what rogue glide files are?

check the mk4 folder (display hidden files must be enabled), and delete glide2x.dll (glide.dll, glide3x.dll) if present.
